Defect Summary:
THE SINGLE STRAP SLIDE THAT IS POSITIONED ON THE RIGHT AND LEFT SIDE OF THE UPPER HARNESS MAY SLIP.

Consequence Summary:
SHOULD THE SLIPPAGE OCCUR THE SYSTEM WOULD FAIL TORESTRAIN THE CHILD.

Corrective Summary:
HARNESS WILL BE REPLACED WITH A DOUBLE SLIDE LOCKING SYSTEM.
